Does every image contain sensitive metadata?
No. Many images contain no EXIF at all. Cleaning is useful when you want a new copy that does not preserve readable embedded fields.
Is the original changed?

The pixels are encoded again. Compression efficiency and quality settings differ from the software that created the original.
